If you're a new client, please read and complete the following forms and bring them to your first session.

• Adult Intake Form or
• Child Intake Form
• Notice of Privacy Practices
• Receipt of Privacy Practices
• Service Agreement 
• Minors and Parents (for children clients only)

Please complete the release of information form if you would like me to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.).  You will also need to complete the form if you would like to utilize Medicaid or other benefits from a third party payer.
